Medical case management is a proactive process in which highly skilled, registered nurses collaborate closely with the patient, family, health care providers, and payor to identify and achieve functional and timely outcomes. SCM does this by coordinating health care resources from the intake of a case referral through a patient’s maximum medical improvement.
